This detail is from a “Gem” Roller Organ. The pins on the barrel open little valves so air can pass through the 20 reeds. This table-top model is just 8” tall and 14.5” across, and you really ought to click the via link below to see the photos of the whole thing and hear a little mp3 sample of it playing.
(Bonus fact from the page: apparently the barrel with pins on it is called a “cob” by people in the know. I assume that’s only true of these corn-sized barrels, but who knows!)
